Resumen
En este trabajo se analiza la documentación publicada por el Organismo Internacional de Energía Atómica (OIEA) referida a la seguridad en los centros de radioterapia y los distintos enfoques de los mismos. En base a las recomendaciones de la OIEA, se realiza un análisis de riesgos, mediante matriz de riesgos, del servicio de braquiterapia de alta tasa de dosis del
Centro de Medicina Nuclear y Radioterapia “Pte. Dr. Néstor C. Kirchner” (CEMENURNK), de la ciudad de Formosa, Argentina. Este servicio posee un equipo de carga diferida MultiSource de marca Bebig, que utiliza una fuente radiactiva miniaturizada de 60Co. El resultado del análisis evidencia la necesidad de crear algunas barreras de seguridad que permitan reducir la probabilidad de ocurrencia de accidentes y, con esto, alcanzar niveles de riesgo bajos. También se incluye el desarrollo de estas barreras. En primer lugar, la implementación de un sistema de cálculo independiente basado en el protocolo AAPM TG-43, cuyo objetivo es verificar los resultados generados por el TPS. Asimismo, se incorporan los registros de procedimientos de validación de la correcta identificación de canales y catéteres en implantes intersticiales. Adicionalmente, se incluyen con fines académicos tres acciones complementarias: determinación de la tasa de Kerma en aire, validación de los resultados mediante un segundo conjunto dosimétrico y la implementación de un sistema de dosimetría in vivo para garantizar la precisión durante los tratamientos.
This work analyzes the documentation published by the International Atomic Energy Agency (IAEA) regarding safety in radiotherapy centers and their different approaches. Based on the IAEA recommendations, a risk analysis is carried out, using a risk matrix, of the high dose rate brachytherapy service of the Center for Nuclear Medicine and Radiotherapy “Pte. Dr. Néstor C. Kirchner” (CEMENURNK), from the city of Formosa, Argentina. This service has Bebig brand MultiSource afterloading equipment, which uses a miniaturized 60Co radioactive source. The result of the analysis shows that it is necessary to implement some safety barriers that allow reducing the probability of accidents occurring and, with this, achieving low risk levels. The development of these barriers is also included, among which is the implementation of an independent calculation system, based on the AAPM TG-43, that contrasts the results delivered by the TPS, a register to be used in the verification procedure for the identification of channels and catheters in interstitial implants and, although it was not mandatory, the determination of the Kerma rate, validation of the result with another dosimetric set and implementation of in vivo dosimetry was done for academic purposes.
